Ultherapy Results: When They Appear and How Long They Last

"When will I see results, and how long will they last?" It is the question we hear most often about Ultherapy. This guide answers it using the published clinical data, and explains what influences your result.
Why people consider Ultherapy
Most patients start thinking about Ultherapy when skin no longer feels as firm as it used to. From the late thirties onward, home care alone may do little for the jawline or the softening of the mid-face. Patients describe it as "my whole face feels like it is sagging" or "my jawline is not as defined as before". Ultherapy is chosen because it lifts without cutting, injecting or downtime.
How Ultherapy works
Ultherapy uses high-intensity focused ultrasound (HIFU). Like a magnifying glass focusing sunlight, it concentrates ultrasound energy at a set depth under the skin. Treatment depths are 1.5, 3.0 and 4.5 mm, and the ultrasound image sees down to 8 mm. The important part is that at 4.5 mm it reaches the SMAS layer, the sheet of connective tissue between the skin and the facial muscles. This is the layer a surgeon tightens in a facelift.
Ultherapy stimulates this layer without an incision. The tiny thermal points trigger new collagen production from the inside, while the skin surface is left untouched. That is why it can improve laxity at depth with no visible wounds.
Published clinical studies report increased dermal collagen after treatment and improvement in brow position and jawline definition. The degree of change varies from person to person.
When results appear
Ultherapy is gradual, not instant. Here is the typical timeline we explain in the consultation room:
| Timing | What you may notice |
|---|---|
| Right after | Slight tightening from the initial contraction of collagen. Mild redness or tenderness for a few hours to a day. |
| 2–4 weeks | Early firmness. Some patients notice the jawline first. |
| 2–3 months | The main change. New collagen has formed and the lift becomes visible in photos. |
| 3–6 months | Results reach their peak and stabilise. |
Because the change is gradual, we recommend taking a photo before treatment. Patients who see themselves every day often underestimate the difference until they compare.
How long results last
For most patients results hold for about 12 to 18 months. Some keep a visible improvement for up to two years. The variation comes from age, skin thickness, sun exposure, weight changes, smoking and how many shots were placed where. A repeat session is usually planned when the lift starts to fade rather than on a fixed schedule.
What decides your result more than the machine
- Shot plan. The number of shots and, more importantly, where they are placed. Concentrating density on the areas that actually sag matters more than a large total number. See our guide to 300 vs 600 vs 1000 shots.
- Depth selection. Ultherapy Prime images the skin in real time so the doctor can watch the image and take care over the layer being treated and the structures around it.
- Genuine transducers. Each transducer has a fixed number of shots. At our clinic we use genuine transducers, and you can check the brand and the remaining-shot counter.
- Realistic candidate. Ultherapy is for mild to moderate laxity. Heavy sagging with a lot of loose skin may need threads or surgery, and we will say so.
Who should wait or choose something else
- Pregnant or breastfeeding patients.
- Active skin infection or severe acne in the treatment area.
- Metal implants or pacemakers near the area, or a bleeding disorder.
- Very thin faces with little fat, where a different depth plan or a different treatment may be safer.
Frequently asked questions
- Does Ultherapy hurt?
- You feel brief heat and a deep prickle at each shot. Numbing cream is applied for about 30 minutes beforehand, and Ultherapy Prime allows lower energy at more points, which many patients find easier than older devices; pain tolerance varies.
- How long does the session take?
- About 45–90 minutes including numbing, depending on the number of shots.
- Is there downtime?
- Usually none. Mild redness or tenderness settles within a day. You can fly home the same day.
- Can I combine it with other treatments?
- Yes. Thermage for surface firmness, Onda for lower-face fat, or a skin booster are commonly planned around Ultherapy. The order and spacing are decided at consultation.
- When should I repeat it?
- When you feel the lift fading, typically after 12–18 months. Some patients choose a smaller maintenance session earlier.
