Thermage FLX: 7 Things to Know Before You Book, Including How It Differs from Ultherapy

"Is Thermage better than Ultherapy?" "Does one session really do anything?" These are the two questions we hear every week about Thermage. Here is how we answer them in the consultation room: what Thermage is, how it differs from Ultherapy, what the evidence says, and who we advise against it.
1. What Thermage is
Thermage delivers monopolar radiofrequency (RF) energy deep into the skin. The heat makes loosened collagen fibres contract immediately and triggers new collagen production over the following months. It is non-invasive, with no needles or incisions, and is one of the best-known treatments for skin firmness and early laxity.
2. What the heat does inside the skin
Collagen, the scaffolding of firm skin, responds to heat. When the dermis is warmed to the right temperature, slack collagen tightens at once, and over two to six months new collagen is laid down, firming the skin from within. Thermage is designed to heat a broad area of the dermis evenly while the surface is cooled, which is why patients see a small immediate tightening and then a slow improvement.
3. How it differs from Ultherapy
| Thermage (radiofrequency) | Ultherapy (ultrasound) | |
|---|---|---|
| Energy and depth | RF, mainly heating the dermis over a wide surface | Focused ultrasound, mainly reaching deeper layers such as the SMAS point by point |
| Best for | Skin-surface firmness, fine lines, texture | Deeper sagging, jawline lifting |
| Combination | They target different depths, so they are often planned together ("Ul-Thermage") | |
In short, Thermage firms the skin itself; Ultherapy lifts the deeper layer. Which one you need depends on how deep the laxity sits, and that is what we check at consultation.
4. What changed with the FLX generation
The current FLX generation uses a larger tip, so a full-face session is shorter. Energy is adjusted to the skin's response in each area, and a vibration handpiece reduces discomfort. The same brand name can give a different experience depending on the device generation, tip size and how the session is planned.
5. How strong is the evidence
Thermage is FDA-cleared for non-invasive treatment of wrinkles and has been studied for nearly two decades. Published studies consistently report modest but measurable tightening in most patients, with results building over several months. It is not a substitute for surgery, and studies also show that results vary more than with a facelift. We explain the limits as clearly as the benefits.
6. What to expect: results, duration, downtime
- Results: a slight immediate tightening, then gradual improvement over 2–6 months.
- Duration: typically 12–24 months. Many patients repeat once a year or every other year.
- Downtime: usually none. Mild redness for a few hours; makeup can be worn the next day.
- Session: about 60–90 minutes for the face; the eye area is treated with a separate small tip.
7. Who we do not recommend it for
- Pregnant or breastfeeding patients.
- Pacemakers, implanted defibrillators or metal implants in the treatment area.
- Severe sagging with a lot of excess skin, where threads or surgery are more realistic.
- Active infection or inflammatory skin disease in the area.
On the day
- Come with clean skin if possible; we remove makeup and jewellery at the clinic.
- Tell us about retinoids, blood thinners and any other medication, and about recent fillers or threads. Do not stop prescribed medication on your own.
- Plan about two hours for a first visit including consultation.
Frequently asked questions
- Thermage or Ultherapy, which is right for me?
- Surface laxity and fine lines point to Thermage; a heavier jawline and deeper sagging point to Ultherapy. Many patients in their forties benefit from both, planned a few weeks apart.
- Does it hurt?
- You may feel heat and some stinging; pain tolerance varies. The FLX vibration and cooling make it tolerable for many patients without injections; numbing cream is available.
- When do results appear and how long do they last?
- Slight tightening right away, main results at 2–6 months, lasting roughly 12–24 months.
- Is one session enough?
- Thermage is designed as a single-session treatment repeated when results fade, not a course.
- Does it work for heavy sagging?
- Only partly. For significant excess skin we will suggest Aptos threads or a surgical consultation instead of overselling an energy device.
